Online Pre-Registration Forms Please contact us at 423-664-4460 if you have any trouble downloading the file. You must have Adobe Reader on your system to view and print the documents. If you do not have Adobe Reader, please see the link below to download to your computer.  1. Click on the registration form below. Once opened, select "File" then "Save As" and save the document in a convenient location on your hard drive. Complete the form.
 2. Once form is complete, RESAVE the form on your hard drive. NOTE: We will ask you to sign and date the form when you arrive for your first appointment 3. Type your name below, then browse and select from your hard drive your completed registration form. Click Submit.
